To receive your personalised report, complete the online questionnaire. You will be asked questions about your general health, lifestyle, blood pressure and cholesterol readings and current medication and treatment. Accurate readings of your blood pressure and cholesterol are important components of this questionnaire and are required for an accurate assessment.
After you’ve finished the questionnaire, you’ll get immediate access to your personalised report and recommendations. You can download it as a pdf, print it off or send to yourself via email. You can also discuss your results with a team of healthcare professionals whenever you need to. They’re on hand to help you.
